Exosome Therapy in Miami: How It Differs From PRP and Where the Evidence Stands

Patients who have already tried PRP often arrive asking about exosomes. The question is usually some version of the same thing: if PRP works by delivering growth factors, and exosomes are supposed to carry an even richer set of signals, is this simply the better version?

The answer is more interesting than yes or no.

What an exosome is

Cells communicate constantly, and one of the ways they do it is by releasing tiny membrane bound packages into the space around them. Those packages are exosomes. Each one carries proteins, lipids and genetic messengers, and delivers them to neighboring cells.

Think of a cell as sending instructions rather than materials. An exosome is the envelope those instructions travel in.

The therapeutic interest is straightforward once you see it that way. If aged or damaged tissue is receiving fewer and weaker repair instructions, delivering a concentrated set of well written instructions may help the tissue respond.

How this differs from PRP

Platelet rich plasma is drawn from your own blood, spun to concentrate the platelets, and reapplied. Those platelets release growth factors at the site.

  • PRP is autologous, meaning it comes from you. That is its biggest advantage: no compatibility question, and a cost that is essentially the procedure itself.
  • PRP quality varies with the patient. Age, health status, medications and even hydration on the day of the draw all change what ends up in the tube.
  • Exosome preparations are standardized products rather than something drawn from you, which means the composition is consistent between patients.
  • Exosomes carry a broader and more complex set of signals than platelet derived growth factors alone.

So the honest framing is not that one replaces the other. PRP is your own biology, concentrated. Exosomes are a standardized signal, delivered.

Where exosomes are being used

Hair

This is where patient interest is highest. Exosome preparations are applied to the scalp, often alongside microneedling to improve delivery, in patients with thinning driven by follicle miniaturization rather than complete follicle loss. Skin

Applied after a treatment that opens a repair window, such as microneedling or laser, with the goal of supporting the healing response and the quality of the collagen laid down.

The part most articles leave out

Exosome therapy is an emerging area, and the evidence base is not yet at the level of an established standard of care for most of these uses. Anyone presenting it as proven is overselling.

Two practical points follow from that.

First, product quality varies enormously. Exosome preparations are not interchangeable, and the difference between a well characterized product and a poorly characterized one is not visible in the vial. Where the product comes from, how it is processed and how it is stored are legitimate questions to ask any clinic offering this.

Second, the regulatory picture in the United States is specific and evolving. A clinic that cannot explain clearly what it is using and under what framework is a clinic to walk away from.

Who tends to be a reasonable candidate

In practice, the patients for whom this conversation makes the most sense are those who have already tried a more established option and want to go further, who have realistic expectations about degree of change, and who are treating a problem where tissue is impaired rather than absent.

Exosomes do not regrow hair from a follicle that is gone, and they do not rebuild cartilage that is no longer there. Signals only help tissue that is still capable of responding to them.

How we approach it

The first step is deciding whether the problem is one that responds to signaling at all. That takes an examination, sometimes imaging, and an honest conversation about what a realistic ceiling looks like for your particular case.

If it is appropriate, we usually discuss it alongside PRP rather than instead of it, because in several applications the combination is more sensible than either alone.
