How Often Each Treatment Should Actually Be Repeated: A Maintenance Calendar

The question we get most often is not what a treatment does. It is how often it needs repeating, and what happens if you stop.

It is a fair question and it is rarely answered clearly, partly because the honest answer is that treatments fall into three very different categories. Once you see which category a treatment belongs to, its schedule makes sense immediately.

The three categories

Some treatments wear off. Some treatments build. Some treatments manage an ongoing condition.

  • Wear off treatments have a defined duration. The effect fades and returns to baseline. Botox is the clearest example.
  • Building treatments stimulate your own tissue. The result accumulates over a series and then declines slowly. Collagen stimulators and microneedling work this way.
  • Management treatments address something ongoing, such as hormone levels or weight. They are not courses with an end date, they are programs with adjustments.

Expecting a building treatment to work like a wear off treatment is the single most common source of disappointment in aesthetic medicine.

Wear off treatments

Botox and other botulinum toxins

Every three to four months for most patients. Treating consistently often means needing slightly less over time, because muscles that contract less tend to weaken. Stopping simply returns you to baseline, and the lines do not come back worse. See Botox in Miami.

Building treatments

Sculptra

An initial series of two to three sessions spaced roughly six weeks apart, then results that continue developing for months. Maintenance is usually annual or every 18 to 24 months. Read more on Sculptra in South Miami.

Microneedling

Three to six sessions four to six weeks apart for texture and scarring, then maintenance every four to six months. Results appear at weeks three to twelve rather than immediately.

PRP for skin

Three sessions four to six weeks apart, then once or twice a year. See our approach to facial biostimulation.

Thread lift

A single placement, with results typically holding 12 to 18 months. The first month of aftercare matters more than most patients expect.

PRP for hair

Three to four sessions at monthly intervals, then maintenance every three to six months. Hair responds slowly, and three months is the earliest reasonable point to judge. More on hair restoration.

Body contouring with HIFEM and radiofrequency

A course of sessions over several weeks, then maintenance every three to six months to hold the result. See HIFEM plus RF body contouring.

Management programs

Medical weight loss

Not a course. Dosing is titrated, labs are monitored and follow up continues as long as the program does. What happens after the medication is the part worth planning from the start. See medical weight loss and tirzepatide.

Hormone support

Labs first, then adjustment, then repeat labs. Intervals depend on the individual and on what is being supported. See hormone therapy.

IV nutrient therapy

Varies widely by purpose. Recovery and immune support are episodic. Correcting a documented deficiency follows a defined schedule with retesting. See IV vitamin therapy.

Regenerative pain management

Assessed by response rather than by calendar. Some patients need a single series, others benefit from periodic treatment. See pain management.

A realistic annual rhythm

For a patient combining several of these, a year often looks roughly like this:

  • Botox every three to four months, so three or four times a year
  • A microneedling or PRP series once, usually in the cooler months when sun exposure is easier to control
  • A collagen stimulator series once, with the effect carrying through the year
  • Body contouring maintenance two to four times
  • Ongoing programs, if any, on their own schedule

That is a rhythm, not a prescription. The right plan depends entirely on what you are treating.

What happens if you stop

Nothing dramatic, and this is worth saying plainly because the fear of dependency keeps people from starting.

Wear off treatments return you to baseline. Building treatments leave you with the collagen that was built, which then ages normally from that new starting point. Management programs are the only category where stopping requires an actual plan, and that plan should be part of the conversation before you begin.

Building your own schedule

The useful version of this article is the one written for your face and your goals, not the general one. That means knowing which of your concerns are movement, which are volume, which are position and which are skin quality, because each answer points to a different category and therefore a different rhythm.

Dr. Ernesto Diaz, a board certified physician with more than 33 years of practice, builds that plan at the consultation. Book yours or see the full range of treatments at our South Miami clinic.
