Botox or Dysport: How a Physician Chooses Between Them

Patients rarely ask for Dysport by name. They ask for Botox, because Botox has become the generic word for the whole category, the way people say aspirin or ask for a Kleenex.

There are several botulinum toxin products approved in the United States, and while they do the same fundamental thing, they are not interchangeable in practice. Here is how the choice is actually made.

What they have in common

All of them are purified botulinum toxin type A. All of them work by blocking the signal between nerve and muscle at the injection site, so the muscle contracts less forcefully. All of them are temporary, and all of them wear off as the connection is restored.

None of them fill anything, and none of them lift descended tissue. If that is the concern, the conversation belongs with a thread lift or a collagen stimulator such as Sculptra instead.

Where they differ

Onset

Dysport tends to begin working somewhat faster, often within two to three days, where Botox more commonly shows effect at three to five days. Both reach full effect around two weeks.

For a patient with an event in a week, that difference is worth considering. For most patients, it is not the deciding factor.

Spread

This is the meaningful one. Dysport diffuses slightly more from the injection point. That is an advantage across broad, flat muscles such as the forehead, where a smoother distribution can look more natural. It is a disadvantage in areas where precision matters and unwanted spread produces problems, such as around the eyebrow, where diffusion into the wrong muscle can drop the lid.

Units

The units are not comparable between products. A Dysport unit is not a Botox unit, and comparing prices per unit without knowing that leads people to conclusions that are simply wrong. Compare the price of treating an area, not the price of a unit.

Duration

In practice both last three to four months for most patients. Individual metabolism, muscle strength and how often the area is treated matter more than the brand does.

How the choice is actually made

In our clinic the decision is usually driven by the area and by the patient history.

  • Broad forehead with fine, evenly distributed lines: Dysport often distributes more smoothly
  • Precise work near the brow or around the eyes: the more contained spread of Botox is usually preferable
  • Strong, well developed muscles, common in patients who frown heavily or clench: the product matters less than the dose and the mapping
  • A patient who has felt a previous treatment wore off early: sometimes a product change helps, and sometimes the real issue was under dosing
  • A patient who felt a previous result looked frozen: that is nearly always a technique and dosing issue rather than a brand issue

That last point deserves emphasis. Patients who dislike their result almost never had the wrong product. They had the wrong map.

Why the injector matters more than the label

Two things determine whether a botulinum toxin result looks natural: which muscles are treated, and how much goes into each one.

Faces do not move identically. The muscle that lifts one patient eyebrow may be doing very little in another. Treating a standard set of points at a standard dose produces standard results, which is exactly the frozen look people are trying to avoid.

Practical notes for either product

  • Avoid lying flat or exercising heavily for about four hours afterward
  • Bruising is possible and usually minor. Stopping non essential blood thinning supplements a few days before, with your physician approval, reduces the chance
  • Do not judge the result before day fourteen
  • Small asymmetries at two weeks are common and are corrected with a touch up, which is normal rather than a failure
  • Treating consistently over time often means needing less, because muscles that contract less tend to weaken

The honest bottom line

For most patients and most areas, the difference between these products is smaller than the difference between two injectors. Choosing a clinic on the basis of which brand it carries is optimizing the wrong variable.
