Ojeras hundidas: relleno u otra solución completamente distinta

Tired looking eyes are one of the most common reasons patients come in, and under eye filler is often what they ask for. Sometimes it is exactly the right treatment. Just as often the cause of the tired look is somewhere else, and filler placed under the eye makes it worse. Telling the two apart is the most important part of the visit.

What actually causes a tired under eye

Several different problems produce a similar look in the mirror:

  • A true hollow, called a tear trough, where volume has been lost between the lower eyelid and the cheek.
  • Puffiness from fat pads that push forward with age, which creates a bulge with a shadow beneath it.
  • Thin skin that shows the color of the muscle and vessels underneath, giving a purple or dark tone.
  • Pigmentation in the skin itself, common in many skin types and often inherited.
  • Loss of cheek volume, which removes the support beneath the lower lid and makes the hollow look deeper.

Only the first of these is directly treated by filler under the eye. The last one is often better treated by filler in the cheek. The others need different approaches entirely.

Who is a good candidate for under eye filler

The best candidates have a visible hollow with reasonably firm skin, little or no puffiness and good cheek support. In this group a small amount of hyaluronic acid, usually placed deep with a blunt cannula, can soften the shadow and make the face look rested. The amount is small, often well under one syringe for both sides.

When filler is the wrong choice

Under eye filler tends to go wrong in three situations. When there is significant puffiness, filler adds volume next to a bulge and the area looks swollen. When the skin is very thin, filler can show through as a bluish tint. And in patients prone to fluid retention, hyaluronic acid can attract water and leave persistent swelling. In these cases a physician should say no, or suggest treating the cheek instead.

Alternatives that work for other causes

When the problem is thin or crepey skin, collagen stimulation helps: microneedling adapted to the delicate under eye area, often combined with PRP, can thicken and brighten the skin over a series of sessions. When pigmentation is the main issue, skin care and treatments aimed at pigment are the right path. When the cause is cheek volume loss, restoring support in the midface often improves the under eye more than treating it directly. Prominent fat pads are a surgical question, and an honest consultation will say so.

What the treatment looks like

When filler is appropriate, the area is numbed and the product is placed slowly, in small amounts. Mild swelling and possible bruising settle over several days. The result is assessed at two weeks. Under eye filler can last a year or more, because the area moves less than the lips.

Safety in a delicate area

The under eye region has important vessels and very little margin for excess product. Anatomical knowledge, conservative amounts, a cannula where appropriate and having the dissolving enzyme available are essential. This is not an area to choose on price.
